At CenterPointe Hospital, our aftercare services can help provide the ongoing support each patient may need to continue achieving success after they leave our facility.

Discharge Planning

Our staff begins the discharge planning process for each person the moment they are admitted to one of our facilities. We continually monitor each patient as they advance through treatment, and we update their aftercare plan along the way to reflect the progress they have made.  

We take a comprehensive approach to discharge planning by involving the patient’s full multidisciplinary treatment team, which may include the physician, therapist, nursing staff, dietitian, and recreation therapist. We also collaborate with the individual and their family to tailor aftercare plans so that they can optimally support the patient’s specific needs and goals.  

Our staff works to customize each discharge plan so that no two plans are the same. Common follow-up therapies, programs, and resources that we may recommend as part of a patient’s continuing care plan include:  

  • Our intensive outpatient program (IOP)  
  • Transcranial magnetic stimulation (TMS) 
  • Private therapy 
  • Senior facilities  
  • Aftercare support groups 

About Our Aftercare Support Group

Our commitment to the well-being of our patients does not end with their completion of our programming. We understand that recovery can be a lifelong process, and we are passionate about helping those who come to us for services achieve continued success long after they leave our care. 

We proudly offer aftercare support groups for individuals who have completed treatment in any of our inpatient, residential, or outpatient settings. These group meetings focus specifically on maintaining addiction recovery, and they may cover a variety of topics, including preventing relapse, finding a sense of purpose, and sustaining healthy relationships.  

Group sessions can support recovery by giving people a chance to continue learning and practicing coping skills that can help them resist future urges to use substances. Meetings can also provide excellent opportunities for individuals to form meaningful bonds with others who may empathize with what they are going through.  

Our aftercare support groups occur twice a week on Tuesday and Thursday evening from 6 p.m.7 p.m. People who are navigating recovery after completing treatment at one of our facilities can attend these sessions for life. We offer groups at no cost to the individual, and prior RSVP is not required to join.  

Sessions are held at CenterPointe Hospital, which is located at 4801 Weldon Spring Parkway, St. Charles, Missouri 63304. When you arrive at our facility, simply check in with the receptionist at the main entrance, and they can direct you to the group meeting location.
